If a parent will be volunteering (chaperone, roadie, otherwise) and will be stepping onto the field at Ford Field, we need the appropriate release and agreement forms forms. If you aren’t sure, you should fill it out.
Also, if a parent is chaperoning for ANY away trip, including states, Indy, or any local shows, we need a Chaperone Excursion form completed as well.
We need a Ford Field Form (Waiver of Liability) for EVERY SINGLE KID in the marching band, all on their own page (no siblings together, etc).
